Life Care Planning  with Advance Directives, Health Care Directives and Living Wills   Provided by the Arizona Attorney General's Office, this website makes available these planning documents and instructions and gives important additional information, links and resources on life care planning.

Arizona Advance Directive Registry   Advance directives (Living Will, Medical Power of Attorney, and Mental Health Power of Attorney) can be registered, stored and easily retrieved online (with your password) when entered in this FREE database provided by the Arizona Secretary of State.
